The multiple exposure is unique, but IMHO it provides too much distraction from the tiny portion of this image that actually shows your face.

Artistic. Was this done in-camera? or is it composed of Photoshop layers? Doesn't matter...I'm just curious.

Before someone says, "Obviously, dummy, it wasn't done in camera--look at the edges of the film," I can immediately tell you how to create this image in camera, on film, with no Photoshop manipulation whatsoever.

I'm giving the artist lots of credit here.
