Striking image. How many arms did he have?!

Many hands on one guitar neck! I find that this image does not have a focal point that adds meaning to the image other than the in focus thumb. What story is that telling in the image? I use a very shallow dof often, but I'm trying to figure out why the photographer made the thumb choice here. Also more contrast in the lighting on the fingers would have made the thumb less prominent.
