Let me be among the first to say, "Yuck." This is a powerful and disturbing photograph that causes me some discomfort. The flash on camera lighting adds to the documentary/forensic/medical/anthropological feel.

What were intentions? (do I have to ask what this is?)

I think it would be better without the head on flash... some glare and the harsh shadows make this image less than in could have been. Since your subject isn't going anywhere, you could try using ambient light and a much longer shutter speed... or bounce your flash off something, etc.

Another thing is where the focus is... I find it a bit distracting that the finger tips are out of focus.
